The headstone to George Perkins, dated 1794, is located 3 meters south of the porch of the Parish Church in Affpuddle. It features a shaped top and a bas-relief of an urn flanked by two cherubs. This headstone is illustrated in the Royal Commission on the Historical Monuments of England Survey, Dorset Volume II.
